At Hogan Care, we understand the challenges faced by individuals who have experienced an accident resulting in disability. Our mission is to provide comprehensive support that empowers our clients to regain independence and improve their quality of life. Our unique Rapid Discharge Program is designed to facilitate a seamless transition from hospital to home, ensuring that our clients receive the care and support they need during this critical period.

Our program is a collaborative effort with hospitals to ensure that patients are discharged safely and efficiently within a reasonable timeframe. We focus on bridging the gap between hospital care and home, providing a structured pathway that supports recovery and rehabilitation. Our team works diligently to coordinate with healthcare professionals, ensuring that all medical needs are addressed before discharge.

At the heart of our program is the development of a personalised care plan tailored to each individual's needs. Our Registered Nurse (RN) Team collaborates with hospital staff to create a comprehensive plan that outlines the necessary medical, therapeutic, and support services required post-discharge. This plan serves as a roadmap for recovery, detailing everything from medication management to physical therapy and beyond.

On the day of discharge, our team is on hand to ensure a smooth transition. We coordinate transportation, assist with settling into the home environment, and provide any necessary equipment or modifications to accommodate the client's needs. Our goal is to make this transition as stress-free as possible, allowing clients to focus on their recovery.

Our commitment to our clients extends beyond the initial discharge. We offer ongoing support to ensure that the transition to home life is successful. This includes regular check-ins, access to a network of healthcare professionals, and assistance with any challenges that may arise. We also provide support in finding suitable accommodation if needed, ensuring that our clients have a safe and comfortable living environment.
